Hi fellow H&S practioners,

I am about to review our accident/incident investigation form with a view to improve data/ information capture. Over the last 6 months I have met with our insurance providers who were investigating workplace accidents. They have commented that the current accident investigation form lacks detail and leaves it difficult for them to defend civil claims (yes, we have had a few serious accidents in the past). This is a system which I inherited, having joined the company late last year, and has been on my 'radar' to review since then - have had to prioritise more importnat issues/GAPS.

They have shown me an example of a very in depth form which asks a series of questions - yes, it does ensure A LOT of data and information is captured it seems OTT to me!

Does anyone have any ideas/examples of forms that I use as a basis for review?

if the existing accident investigation system is not capturing sufficient info to satisfy claims management then it is unlikely that the system will be sufficiently robust to identify root causes with the objective of preventing similar accidents.

I have some example forms that are developed to ensure that supervisors / managers collect the necessary facts to enable suitable conclusions and recommendations to be made.

Hi Steve, yes, you are entirely right. TBH, I have been concentrating on proactive steps to improve safety performance which has resulted in some significant improvements.

When I joined the company there was very little in the way of risk assessment, HMSM or SOP's - quite concerning really. We had a rather serious criminal case hanging over the company and many civil claims - it will take some time to tackle all aspects of management............tough going but keeps me out of trouble!
