Is it advisable to set target to report nearmiss for individuals.(weekly minimum 4, a individual should report)
if it is so can any body say the exact definition of nearmiss.

I would not set that kind of target, you might end up getting people reporting things which are not 'near miss' or all reporting the same thing to 'get the numbers up'. Having a target to increase the reporting of near misses generally might work, providing everyone involved knows that they are proactive reporting.
We use a report for Near Miss, Hazard, Unsafe Act, Safety Suggestion and see these are proactive reporting so have a target for employees to report more this year than last year. (achieved)
To encourage employees to report these we have provided training (which included Bird's triangle and an estimate of how many near misses we have had against our accident record). Also we award one report per month £20 in vouchers (most significant/potential/previously missed report wins).
Our Near Miss definition:
A near miss is an unplanned event that did not result in an injury, illness or damage, but under slightly different circumstances could have. Only a fortunate break (luck) in the chain of events prevented injury, fatality or damage from occurring.

In the rail industry, a new system of reporting near misses has recently been introduced. It encourages people to report anything they feel is a near miss, without feeling they will be targeted as a trouble causer. It does not matter how many they report, and they are not expected to report a certain number each month.

A near miss means that something has gone wrong or someone has missed something. Time for a review.
Everyone should be looking out for everyone else (HASAWA) best form of 'reward' is life.
