I know that an employees doctor may with hold information unless consented by the employee. It has now come to the point of needing to request information from their GP and a while back I was led to believe there was a particular format for the employee to complete to send to their GP and likewise for the letter requesting information about our employee. So, the question is: a) is there any particular format for asking employee to approach his GP and Letter to the GP and b) does anyone have examples of such with blanks for names I may view. All assistance much appreciated. Badger

If you're referring to the 'Fitness Note', details are available form the Department of Work and Pensions. Introduced by Dame Carol Black, it's designed to respect the patient confidentiality you refer to while at the same time making it straightforward for (well-disposed) general medical practitioners to indicate where relevant boundaries to a patient's state of health and wellbeing at work have to be respected. Have you sent the g p of the employee a copy of the Fitness Note accompanied by a diplomatically-phrased request? By also sending the medics appropriate 'thank you' notes where appropriate, you may well 'stand out in the crowd' in their minds. I was as surprised as I was amused to hear the positive expressions used about me by both OH and general medical practitioners, simply by treating them with 'normal' courtesy in response to brief consultations which cost 'loadsa money' in some other EU countries (e.g. 35-50 euros for a G P consultation in cities in the Republic of Ireland).

Hi Kieran Nope... Fit Note definitely not alluded to. Just seeking a particular format for asking employee to approach his GP and Letter to the GP and wondered if there's recognized wording to be used. But thanks for your comments. Badger

In that case, perhaps useful to ask staff (e.g. library or CEO's office) of a, the Royal Society of Medicine and b. the Faculty of Occupational Medicine.

Isn't the Access to Medical Reports Act 1988 still enabled? Or am I completely out of date. I'd like to know and am sure Badger would. This does require the employee to agree to approve the report, but often with a supportive employer the employer employee relationship can be assisted by many means, even if under a duty of care the employee has to be removed from the current post. I, forgive my lack of knowledge, would have thought that an employee that is "dismissed" (and I say that for ease not legality), would have brought a good HR history and thus many company's would take kindly to the problem and show their corporate responsibility to the employee. I suspect many would not, however I am lucky to work for an employer that pays relatively low wages but will look out for "work family" and help them in any problem - from broken legs to alcoholism. A past case of mine: A driver for 25 years, no problems to be concerned of in his records, yet has a lung disorder and cannot unload even a case of 2 kg papers without being out of breath. Moved out of Multi Drop (many boxes many trips) to trunker (1 load, assisted with MHE), still showing signs. Eventually for the drivers safety and every ones else - he was driving a class 1 on motorways! we had to involve the GP. Prior to the GP we signed him on 'garden leave' at full pay, and CRITICALLY ensured a dialogue was kept open with him and that he was welcome to come in to the offices at any point to have a chat with other drivers or with management to maintain continuity. This is a real issue and not just for badger. Please don't flame me, I'm Only Tech IOSH so could do with your help. However - if you want Food Safety then let me know and I'll get out the blowtorch!

Please don't flame me, I'm Only Tech IOSH ... Bernie You're not JUST a Tech IOSH, the info you provided me earlier I have found to be very useful and now seek to take that forward. Likewise a big thank you to Zyggy for info supplied. I'm just amazed that there's nothing of the sort that Bernie and Zyggy have supplied to me on their website. Badger

Badger - Until you posted this topic it seemed to have been a few weeks since you were last seen on this forum. Some users may have thought you had either emigrated, opted to hibernate or, worse still, been culled as part of DEFRA's drive against bovine tuberculosis! Also, my missus recently saw one of your relatives strolling across our front lawn late one evening - a bit of a surprise as we live in an urban area. Hope it comes back again soon so I can try to shoot it....... with my camera! Can't be bothered spending long periods waiting and watching for it, so might investigate linking up some sort of alarm or even a camera (if I can borrow a suitable one) to our heat-activated front security light. However, might get a fair number of 'false' alarms from the local cats and mangy urban fox.

Graham Had thought of hibernating but then it turned too warm. The reason why you have'nt heard from me and don't tell any one is I've been filmed in a walk on part for Autumn Watch. They are talking of also doing something for the spring as well but I said they would have to catch as and when. Badger PS TRhanks to everyone who has responded, I now have a better picture of the requirements.
