I know this is a big ask - but can anyone point me towards a format for undertaking a risk assessment. Many thanks in advance.

Try the HSE Publication L8 - "The control of legionella bacteria in water systems. Approved Code of Practice and guidance." http://www.hse.gov.uk/pubns/priced/l8.pdf

You don't specify risk assessment on what? - could be anything from a shower head to a power plant cooling tower !!
