Go to 3M webpage, free training provided you buy their stuff with reusable masks it works out cheaper.

Legal requirement under COSHH since at least 2005, perhaps 2002.
Mandatory for all tight fitting filtering facepeices, 1/2 masks, full face masks, SCBA etc...
Not required for powered "turbo" hoods - dont have a face seal.
Debateably not required where a mask is only provided for the function of controling nusiance odour / dust
Can be done in house or externally via contractor. Kit costs £2-3k to buy and individuals can be trained to do in house.
Can be qualitative or quantitative, former being subjective, later being more accurate.
Not everyone will pass a face fit test for a mask, variety of masks, shapes and sizes may be required.
Individuals with stubble / beards unlikely able to get a good face seal
Face seal can be affected by weight gain/loss, dental surgery, scars, facial hair growth and other PPE
Debatable requirement for a specific period for re-test other than after the aforementioned changes may have occured to an individual (unless specified in specific regs, lead radiation, asbestos etc.) however guidance may soon be issued by the HSE.
Erm did I miss anything?
