After a few years lapse, I need to get back into this subject.
I'm upgrading my confined space training (that I deliver). I've obviously checked the usual sources but was wondering if anyone can point me towards material & information that has impressed them.

When I was Group Safety Manager for a chemical company the HSE Confined Space ACOP was never off my desk. http://books.hse.gov.uk/...alogueCode=9780717662333

It is still unbeatable for planning confined space entries, in my view.

on a similar line it has been proposed we are using 3.5m dia hpde pipes which will require operatives to enter and weld the joints. any pointers i should make sure i dont miss?

IS THERE ANYTHING I'VE FORGOTTEN?
Chronicles the events leading up to an accident in a vessel in a chemical plant. Shows the correct procedure for isolation of the vessel, before a man goes in to do a repair. The accident is ascribed to the failure of the man in charge to guard against hazards from outside sources. Intended for supervisors, middle managers and works personnel.
