INDG175(rev2) mentions the need to provide basic health surveillance in the form of a questionnaire that regularly seeks information about early symptoms of ill health. Is there a standard set of questions?

Sample questionnaires are available in The Guidance on The Control of Vibration at Work Regulations (L140) that can be downloaded from HSE books. Specifically you want appendix 2 for the intial screening form & appendix 3 for annual health survaillance.
