I am looking for some feedback from different industries on how they deal with contractors who infringe site rules or are deemed in breach of health and safety legislation or carry out a dangerous activity. Currently on the site where I work we have a lot of maintenance engineers and persons carrying out new installations. We run a yellow/red card system similar to those run in the construction industry, where you get a yellow card for a minor infringement and a red card for a serious one which also means you are banned from site. We are carrying out a review of the policy and I wanted to know what schemes other businesses run. We are thinking about applying a time limit on the ban on the conditions the individual(s) involved receives further instruction, training, coaching and mentoring and can demonstrate lessons have been learnt and are safe to work on site.

We simply build a zero tolerance into the contract, and if anyone breaks it, they're all 'down the road'. Then we recoup costs for disruption -again a contractual obligation on the supplier to agree to.

Hmm...does the yellow/red card equally apply to the PC's staff and representatives of the Client - I bet they don't! I can't believe there is not a better way of maintaining discipline and safe practices on a construction site.

Yellow red card system is great, if used across the board. The M&E guys are a bit of a law onto themselves. I worked on a job for LU, and yellow cards were issued, and followed by a TBT with the supervisor, and possible re-induction with the supervisor. What age group are the persons in question? The younger age group I find dont really care about anything.
