The relative strength/weakness of a pro-forma is neither here nor there in the grand scheme of things. A suitable and sufficient assessment is the required result and should be the focus for your criticism, advice and support. A "general" risk assessment form (MHSWR Reg3) can serve to achieve that end.

There are some examples of COSHH risk assessments on the HSE web site here http://www.hse.gov.uk/coshh/riskassess/index.htm

Caution with some of the HSE specimen risk assessments. They are not always what they should be. For example one engineering workshop assessment completely ignores COSHH. Presumably they don't use any chemicals at all in that workplace!!! Another, for car body repair shop shows the manager providing natural rubber latex gloves for paint thinners. For the uninitiated completely the wrong glove! True, they do state that these are only examples of how one might approach this, but how many managers in SMEs would recognise the errors? Chris
