I don't know why you are asking for a template, if you assess the risk, then put corrective actions in place, do they need to wear safety glasses, yes or no

For a basic study of what you are providing have a look at the BS/EN codes on the frames and oculars of your specs/goggles and compare them against the information in the following HSE internal ops document.
http://www.hse.gov.uk/fo...s/fod/om/2009/03app3.pdf
This should give you some indication of what you should be providing for the activities you are undertakeing. It is also worth bearing in mind that if you are grinding then goggles should be used rather than specs. Hope this helps.

I agree with previous two posts and would also suggest a look at the Personal Protective Equipment Approved Code of Practice: it has straightforward guidance.

Are they asking you to assess where you would need to differentiate between LEP, Goggles or Full Face Sheild?

Even with normal light eye protection it is possible to get small particles of grit/dirt in the eye, especially in dusty environments like construction sites. That said, one could be walking down the street and get something in the eye from passing traffic or whatever. Do we accept this or are we saying that maximum protection must be applied at all times?

Epideave. The "let's all wear eye protection whether we need it or not" idea is very common in the US (do you work for a US-run company?) and personally I don't find it very helpful. I think it devalues PPE to force people to wear it just because someone thinks everyone should do it rather than because there is a real risk of eye injury.
In your shoes I'd be listing all the relevant tasks on site and looking at the actual risk of eye injury on each (use severity and likelihood matrix or whatever other mechanism you usually use for assessment rating). Then assign the correct protection to each task as others have already said.
Template could be a simple table - Task, hazard, risk presented by that hazard, assessment of magnitude of risk, type of PPE recommended, other action required to reduce risk. Doesn't need to be anything fancy.
