Hi All Looking for information on mast climbing near misses. I am investigating a near miss involving a mast climber that was brought down too fast and by passed the 1st and 2nd breaking limit then hit a wall parapit and scaffolding.I have too write my conclusions and recommendations.Some of my conclusions were operator error,not working to there RAMS.Some of my recommendations were refresher training and relaying all tool box talks again. Any more information would be appreciated.

D best of luck After undertaking many investigations I advise that you only put down on paper the facts and nothing else especially speculations and get as many first hand statements as you can As for recommendations etc. I tend these days not to provide any as such things as part of the report as these areas can actually be used against the employer
