I'm not sure what your after. Are you looking from an occupational health view (audiometry, HAVS assessment, spirometer etc) or occupational Hygiene (vibration, noise, etc)?

Hi Derek,
I may have some old Toolbox Talks and example questionnaires I used to use for low risk stuff. If ou want them let me know and I'll dig them out. Might be a couple of days though as they are packed up at home in my home office! They may be of some use.

Do you mean an assessment process to decide what health surveillance is needed? Or something else?

The company involved want to go for there Chas Accreditation antd they are looking for health surveillance assessments for the various disciplines or can I use a generic one as they were looking for the items I listed.

The short answers is yes we do. Thank you.
The longer answer is that you need to talk to an OH provider who can advise you on what you might need. The driver should not be CHAS (or what ever scheme) but the actual requirements of your staff. Health surveillance should only be carried out if
• The work is known to be hazardous to health in a particular way.
• The is a valid* technique for detecting and measuring this health hazard.
• Damage to health is likely to occur under the particular conditions that occur in the job in question.
• That surveillance can be used to establish controls, which will benefit the employee concerned.
