Zanshin be very careful to ensure that your meet the minimum requirements on all the PPE or you will be both in breach of the law and run the risk of a claim against your company if anything goes wrong.

Bob is wise to caution you about the design of any intervention PPE you undertake on behalf of your employer (or client organisation). Having spent several years as an employers' rep in an organisation where Norman Tebbit (now the 'Lord') was the employers' adviser, I learned at first hand how any notion of posing as a leader in negotiations with trade union folk is a risk to avoid!
As the legal and employee relations consequences of simply borrowing a form in the hope it fits situations you haven't even outlined could be very time-consuming and costly, why not do the professional thing? Simply analyse the risks you identify and test out alternative versions of the form.
Essentially, it's an ergonomic exercise of matching risks you identify with vulnerable parts of the body of employees, which are not otherwise protected.
If you'd like a couple of relevant references, you're welcome to PM me with an indication of the settings in which PPE is needed, and the reasons why, and I'll forward a couple of references about relevant necesssary features of ergonomic design to include in an evaluation form, for discussion with trade union representatives.

He says union "members", nothing about any union reps, or even the union itself.
Does the union even know ?
Or is it another "50p face mask versus £250 quid powered rpe"
I, for one, shall never forget starting work for a company and finding the paint sprayer using a cotton-wool and gauze "mask" for spraying zinc chromate paint.....
I would also like to know the point of the exercise......comfort, cost, cosmetic or ?
