Hi All wonder if you could help, I have been tasked with trialling a number of PPE items in consultation with union members, however I have been asked to record my findings eg rate on a scale of 1 to 5 etc does anyone have a form they could share to help with this please, again one that covers fit comfort userbility etc many thanks in advance

Bob is wise to caution you about the design of any intervention PPE you undertake on behalf of your employer (or client organisation). Having spent several years as an employers' rep in an organisation where Norman Tebbit (now the 'Lord') was the employers' adviser, I learned at first hand how any notion of posing as a leader in negotiations with trade union folk is a risk to avoid! As the legal and employee relations consequences of simply borrowing a form in the hope it fits situations you haven't even outlined could be very time-consuming and costly, why not do the professional thing? Simply analyse the risks you identify and test out alternative versions of the form. Essentially, it's an ergonomic exercise of matching risks you identify with vulnerable parts of the body of employees, which are not otherwise protected.
