I am looking for some reading materials in relation to behavioural safety, so if anyone can recommend a good book to read it would be appreciated.

In case you find Dominic's book difficult to get, there are also other readable, well-researched guides to 'behavioural safety' by Dan Petersen, Scott Geller, Terry Sweeny and Tom Krause
