I use a spreadsheet. Page for each person of what and when issued. (sizes inserted where applicable) Another page for recording replacement issues (So I can keep track of who gets through more items than other people) Allows me to also record what items require returning when an employee leaves. I am about to modify it so I can record costs also. (when I get time!)
