Under LOLER thorough examination report: "Records must be kept of all thorough examinations and any defects found must be reported to both the person responsible for the equipment and the relevant enforcing authority." Question: who is the term "relevant enforcing authority" refers to?

It's either the HSE or the local council dependent on who enforces H&S at the premises. However, LOLER doesn't require all defects to be reported to the enforcing authority, only really serious ones. And it's the person doing the examination who has the duty to make this report, not the client.
