All Could someone clarify a question . we have had a D O and is reportable however we are part of a JV but we are not PC at present but the Dangerous Occurrence happened to our sub contractor ? Thanks in advance

SNS - Why subbie ? just curious, my opinion is the duty holder (person in control) which in this case would be the PC.

Hi all I have requested that we should investigate why our subbie has had this happen in the first place and reiterated that the F2805 needs completing as also per instruction from the PC. collapse, overturning or failure of load-bearing parts of lifts and lifting equipment; could someone please confirm that lifting equipment would include lifting slings failing ? surely just for my sanity if nothing else !! thanks in advance

Lifting "equipment" is everything up to the hook- anything attached to the hook would be "lifting accessories" David
