Does any kind person have a simple checklist that a line manager could use to ask the right questions when his employee has had an incident please?

The process we use in-company is too complicated in my opinion - what I would like to do is to instigate a checklist that will enable a manager to get answers to all the right questions then I can get involved to take the investigation further and put some actions/lessons learnt in place across the business.

The first question should always be "so, what do you recollect about the incident"? The purpose is to allow the interviewee (injured party or witness) free narrative. Allow them to recall without introducing bias.

Then you ask questions intended to (a) clarify something they said, (b) corroborate something someone else said or some physical evidence already gathered.
