I have just received an email flyer from the magazine "Safety Health Practitioner" It's main headline announcement reads: "A packaging firm has been fined £200,000 after a worker severed fingers on both hands when they became trapped in a machine at a factory in Whitehaven." beneath the most unfortunate headline: ""Six-digit fine for power-press incident""

I bet they meant it. I've complained several times about the distasteful headlines in SHP; I've come to the conclusion such crass statements are part of whatever passes for journalist's training these days. Pity IOSH are smeared by association though.
