We publish all near-misses and resulting actions in a positive way, those reporting get individual feedback and a thanks from their line manager, but the number of reports seem to have reached a plateau and have started to fall off over the past month or two.
So, how can I refresh near-miss reporting in the minds of the (mainly site) staff in an imaginative way, rather than just the standard toolbox talk that includes our couple of real-life examples (which serves its purpose, but they’ve all seen it already)?
I’ve googled the obvious phrases, I’ve searched this forum, but haven’t come across anything “different” yet.
I’m not looking for the “silver bullet (point?)” solution or whatever it’s called (although if you happen to have it ………), but maybe there is a really excellent short video or practical demonstration idea that someone can point me to?
Any off-the-wall suggestions also considered (it is Friday, after all)!

Hi Steve,
You say you get the line manager saying thanks, push it a bit higher by getting the senior management paying a visit and give them a couple of good near miss cards to comment on. It shows that these things go all the way to the top and has senior management involvement.
