what are the main rules/regulations with regards to a lone worker carry out drilling operations from a ladder on a regular basis, should this be a two man operation

I don't think that lone working is the issue here : it seems to be the working at heights.

Whilst I partially agree with the above comment from PH2, there is still an issue when working alone, particularly if it is high risk work. If the person was to fall and get injured, then there might not be anyone about for assistance. Where possible working at height should be a minimum two person operation.
Ideally a robust SSOW should be in place for W@H. This should include when working with ladders ensuring the ladder is stable and properly secured e.g. eye bolt fixed to a wall. The operative can then use a harness and karibiner for 3 points of contact whilst drilling.
Would this be cavity wall insulation work by any chance?

Dav, as I suspected. There is an organisation (the name escapes me) who provide guidance, including best safety practice for the installation of cavity wall insulation.
I am aware from previous experience that the work can be high risk and low paid. Many are paid on a piecemeal basis, therefore there is a tendency to cut corners by not having the ladder properly secured etc. Not intending to get on my high moral horse, it does not make it right. Many use a slip mat and do not secure the ladder, but when drilling 20mm holes into a brick fascia an eye bolt with strap could be part of the SSOW without too much of a burden.
Okay, the question was about lone working. Now that I know it is cavity wall installation and only one person, it is lone working in my opinion.
