Can anyone help with form 2, I am struggling with what is being asked. A 1 line example would be a big help. I don't want to spend a lot of time and shoot off on a tangent

I presume from the question that that you are on the course and doing the submission.
Which text book have you been using? The RMS book has examples on page 191.
Has your provider given you a template? and / or guidance?
I won't give you the answer but could find a blank word layout if you don't have one. PM with an e-mail address.
