Does anybody have an example of part of a TOR worksheet that they would be willing (and able) to share with me. I only need it as an example, so it doesn't matter whether it is blank or filled-in and I only need part and not the whole worksheet.
Any assistance would be gratefully received.

A quick search of TOR (Technique of Operations Review) gives a google books "hit" that may furnish you with sufficient information to build an appropriate q-set.

Also worth a browse through pertinent HSE commissioned Contract Research Reports (CRRs)
TOR has been around since the early 1970s.
