Hi everyone, bit of advice needed.
There has been a serious breakdown in communiction between the HR department and the director responsible for health & safety.
An employee suffered some ligament damage to his shoulder while disposing of some waste wood resulting in him being off work for a number of weeks.
This information has not been passed through the system meaning the H & S director has not done a RIDDOR to the HSE.
Should he still do one even though it is way over the 15 day limit and face the consequences or not report it due to the time lapse.

Yes- and there are unlikely to be any consequences as you are reporting from date of knowledge.
I would ensure that it can't happen again in case you get asked what you have done to rectify an accident.

Yes report it, I have reported late previously for a similar reason. look at improving your current systems to prevent this happening again.

Yes you should report it. In my experience the HSE use reporting as a statistical gathering tool as much as anything else. They will decide on a case by case basis whether or not to investigate the issue. They are well aware of admin and communication issues and would rather have it reported than not. As mentioned, the improvement of internal systems is a must to ensure it doesn't happen again.

Definately report then review the systems, much better than not reporting and then it coming out several months later
