How often do fire extinguishers need to be checked, 12 monthly or more often? Thank you

What do you mean by 'checked'?
Do you mean servicing? If so, then I believe the BAFE schedule (and British Standards) indicates annually +/- 1 month.

Fire extinguishers must be serviced annually from their installation date and every 5 years they must have an extended service which includes a discharge test and internal inspection. CO2 Extinguishers have a shelf life so they are not subjected to a discharge test.
They must also be regularly checked to ensure they have not been damaged, discharged or lost pressure etc
At my place we inspect them monthly for the points listed above and record the findings.

Do you ever find anything wrong on your inspections? - seems a bit OTT to me

Have a look at this website www.safelincs.co.uk/bfm P50 Service Free Fire Extinguishers that have a ten year warranty and a 20 yet life cycle. No I don't work for them - don't even use them yet, but I am looking at their Kevlar extinguishers

As ever, the devil is in the detail; if you ask the right question you have a reasonable chance of getting the right answer.
I have always understood that formal inspection and servicing is annually however, ‘checks’ or inspections may well need to be carried out more frequently depending on the situation.
If, for example you are the occupier of a premises that has a history or fire equipment being tampered or interfered with, damaged, discharged or going AWOL then I would hope that it is reasonably obvious that an annual ‘check’ (to see if the extinguisher is actually there, undamaged and not discharged) would be insufficient!
