Several years ago I used an HSE safety culture survey, NOT the electronic version, but one contained in guidance. It had the questionnaires and how to score and interpret the results. I cannot find it now, any clues, or reference numbers?

I am only aware of the HSE's Safety Climate Survey which came at quite a cost. That said, I do recall reading a HSE document some years back which may have had examples. Suggest you go to the HSE website and search 'safety climate survey' and you will find a rake of documents - too many to list.

I have designed and implemented several culture surveys so I would be happy to advise or provide examples.

It is useful however before you start it think what you are going to do with the results. If you run it and tell people that there are problems, then they will ask you what you are going to do about it. Problem is, safety culture is very hard and slow to change................
