LOTO is a USA OSHA term so you may want to look up the https://www.osha.gov/SLTC/controlhazardousenergy/ website

smart, in UK this would refer to appropriate isolation of a machine, its electrics, compressed air etc. e.g. when cleaning, repairing, routine maintenance, clearing blockages etc In the PUWER regs it is reg 19 'Isolation from sources of energy'. There's some explanation in the ACoP L22 and there is a complete guide from the HSE in HSG253. Start your learning here http://www.hse.gov.uk/safemaintenance/permits.htm There are various systems of padlocks and tags available to do this, and a permit to work system will often be appropriate too.
