Hi Guys, Im currently looking at health surveillance for our workshop employees and have been advised that low level surveillance can be achieved from a simple questionnaires. Does anybody have a sample I could have a look at. Yep i know its a bit cheeky. Thanks

I don't think you should try to do this yourself - better to get an occupational health provider to do it for you. They will be able to make sure that the right questions are asked, preserve medical confidentiality, and advise what you should do when you get answers indicating problems. Because the questionnaire alone won't achieve anything - you need a way of dealing with the answers to it.

Caution. Who is to complete the questionnaires? If, as I frequently encounter, the intention is for the questionnaire to be completed by the actual employee, keep in mind that the requirement is that the person completing the questionnaire must be competent to do so, i.e. must have the appropriate training to be able to recognise actual health conditions relating to occupation. Will you be able to claim that all your employees are competent? A study commissioned by the HSE on skin conditions among printing workers showed clearly how among those who had claimed no skin problem a significant proportion, when examined by a dermatologist, had work related skin conditions.
