Under OHSAS 18001 is there a requirement that when documents are printed off on paper to have them marked with the appropriate text on each sheet of paper with the words "uncontrolled document"

The answer to that is No. some companies may chose to do this as part of their systems, we don't.

Little bit old fashioned from the days of BS5750 and type writer generated documents. For my employers over the last decade all "controlled" documents" have been held electronically with a page footer that gives the date of printing and a text line "Valid 24 hours from print date unless signed by the Management Representative". Instead of spending days copying, signing and distributing I now have very few paper documents in the wild.
