Hi All,
Been away from the forums for a while but in need of some help.
As a part of my current project I am researching incidents / root causes of lifting equipment and rigging failures in the offshore oil and gas industry globally.
Specifically I'm looking for information, reports, lessons learned, articles, safety flashes or any relevant data where rigging or equipment failure has been a factor in incidents there is a preference towards dropped pipe, or riser assemblies but happy with any data I can lay my hands on.
If anyone has any sources of information or anything they are willing to share I'd be extremely grateful. Happy to share the finished work once it is complete.

I think I may have a flashed incident somewhere - relating to failure of coupling links, I will try and dig it out
But if you are looking for something for a lit review it will not be suitable - No academic weight
