Does anybody have a cold storage and warehouse health monitoring questionnaire that they would be happy to forward on to me.

Richard it isn't really something the untrained person should be doing..happy to share best practice advice, but the checks and how they are interpreted should come from your occupational health provider...

Sorry I am assuming you are not a Occupational Health provider as you would know what to check for and how to record it...
