Can anybody point me in the direction of prosecution of Board Member(s) due to the lack of clearly defined ultimate H&S responsibility within organisation? Thank you in advance.

Try searching the HSE prosecution database for Breaches of The Health and Safety at Work, etc. Act - Section 37. There might be some prosecutions listed there.

Thanks Martyn. Yes, some prosecutions are listed over there, however, they only contain brief description.
