What would constitute a situation where a LOLER inspector would be obliged to inform the HSE of a fault on a machine e.g. telehandler?

This varies.
HSE guidance says that if the fault is such that the inspector decides it is a danger to people and has to remove it from immediate use.
In practice, we had one contractor who reported every fault to the HSE, including lifting accessories that are non-reportable.
