Safety KPIs in your organisation - can you help?

We are about to head in to discussions around our KPIs for the next financial year and I am trying to gather as much information from various groups as I can around current practice.
Would you be prepared to answer the following questions so that I can get an understanding of current practice out there? I don't need company names but if you could include your industry sector that would be incredibly helpful.
1. Do you have Health, Safety and/or Environmental (HSE) KPIs 2. Do you use a mixture of lead and lag indicators or only lead / only lag? 3. What are your specific HSE KPIs (ie actual accident/incident numbers, incident rates (TRIFR or LTIFR), near miss reporting, site visits, audits or other)? 4. Are your HSE KPIs linked to any incentive schemes? 5. If they are (a) what portion of your incentive scheme is aligned to the HSE KPIs? and (b) do you use both lead and lag indicators in your incentive scheme or only lead or only lag
I would be most grateful for any information you are prepared to share.

What are your specific HSE KPIs (ie actual accident/incident numbers, incident rates (TRIFR or LTIFR), near miss reporting, site visits, audits or other)? Incidents reported ( divided into serious, minor and near misses) per FTE plus, number audits and number of actions closed out, training delivered. 4. Are your HSE KPIs linked to any incentive schemes? No 5. If they are (a) what portion of your incentive scheme is aligned to the HSE KPIs? and (b) do you use both lead and lag indicators in your incentive scheme or only lead or only lag See answer for 4

1. Yes 2. Mixture 3. AIR, AFR, SAR, number of accidents. They then are benchmarked against a group of AIRTO companies. The proactive measures are drawn from interviews with employees during our 6 monthly internal audits. 4.No 5.See answer 4

1. Yes 2. Lead and lag measures are reported on 3. Range of things from incident numbers and rates to positive interventions and training completed 4. No - I have seen various schemes over the years - both those that directly benefit employees and those that link to charities. You need to think carefully about what you want to achieve and what the unintended consequences may be of linking incentives to specific KPIs
