What are the key methods used to ensure good standards of health and safety on a construction site.

Firstly you need a very proactive strong client with the right attitude who complies with their own written policies etc. - I find that on average that this does not happen
Secondly you need a very proactive strong primary contractor with the right attitude that will comply with their own written policies - I find that on average that this does not happen
Thirdly you need an enforcer that will enforce properly especially clients - I find that this is a bigger wish than the other two examples given as it does not happen
There are very good but few examples out there of good practice but most examples are poor e.g. all parties need to get the whip out on the senior people not the day to day people and use it from day one!!!

construction work is controlled by contract - which is managed closely by the project managers for contractor and client to ensure the client gets his structure and the contractor gets his money. If Health and safety is adequately written into the contract specification - then it will be managed in the same way as time cost and quality. If the contract focuses purely on time cost and quality and then says simply (at the very last line) 'comply with the H7S@WetcA' or similar vague platitude - then the workers will get injured as the employers chase the time cost and quality.
Whether this counts as a 'key method used' as per the OP request - I don't really know. I do know it is very very rare for H&S to be nailed in contracts... So maybe not a 'key method'.
Is locking the gate at 5.00pm a 'key method'? (sorry it is Friday...)
