What size of construction site are you looking at? A medium to large construction (new house building for example) should be managed through a "Project Management Plan" within which would be a section for all method statements and associated risk assessments for all differeing trades.

ergo with the multitude of trades, and the variety of construction / demolition tasks those trades may be asked to perform there are infinite answers made even more interesting where the full task involves multiple trades at site in either concurrent or consecutive manner

It should simply be a step by step method of how you're going to do the task, a list of equipment to be used, a list of materials to be used, a brief emergency plan (first aiders, nearest hospital, contact numbers), management structure and supported by relevant risk assessments.
Don't over complicate it!
If you're submitting it to a client they will give pointers on what other information they need once it's been submitted.
