I currently work for an organisation in elderly care and have been tasked with putting together an accident reporting system. We want to move away from the accident book but for the moment we won't be introducing an online reporting system like Datix.

I want to develop a accident/ incident reporting form and a separate spreadsheet for each home for them to transfer the data onto so that I can perform trend analysis.

I wondered if anyone already has such a system in place and would be willing to share it with me?
