Hads anyone come across a risk of collision risk assessment before? one of our customers has asked for one and when asked to provide an example could not supply.
they wish us to identify areas where there is a greater risk of collision within the warehouse. This has already been done within the workplace transport ra, equipment specific ra etc but they keep pressing for a risk of collision ra

You could inform your customer RAs are normally task based and therefore the risk of collision is incorporated into the task, which I assume would be - moving pallets/goods with a FLT etc.

Can't help but wonder why does this 'client' want to see this RA?

Agree with RR. A task based risk assessment for the use of fork lift trucks should highlight the risk/hazard of collision.

Never seen a ra simply assessing the collision risk. I guess you could consider specific issues such as blind spots, speeding, driver training, sight lines- but these should be incorporated into your task based risk assessment considerations anyway.

Just a thought.
You are a warehouse, so presumably the client has stock stored within your warehouse.
Are they interested in the collision risk purely from a property damage aspect - damage to their stock?

If so, cover off things like rack guarding systems, rail guidance (if applicable) and prevention of unplanned early departures from the dock. All areas where their stock might be exposed to damage.
