Do you need a separate Permit Form for the type of High Risk Work you are undertaken i.e. one for Hot Works another for Electrical??? Or can they be condensed into one Form?....do the forms need to be as detailed as possible (have you such and such ppe etc), or can you clear a lot of these requirements in sending the Companies Site Rules and asking for Method Statements / Risk Assessments?

I have all of our permit to work on one form. Simplifies the process and as they have to be completed at the site of work they dont need to have a tick box standard questioning to them so they all fit on one form.

so if you were stating the Permit was for hot work, you wouldn't have to tick boxes regarding hot work safety i.e. combustible materials removed from area etc

I have the same permit for every time, but I issue separate permits for roof work and hot work. The reason for this is that they are time dependent. Roof work requires two man working and may be part of a separate process such as maintenance for the air conditioning units. The maintenance may take 3 days with the roof work only taking half a day. I don't want to authorise them to go on the roof at any time during that three day stint, I want to know when they are on, when they are off and that they are safe within a confined period.
Same with hot works. The hot work may be a small part of a larger overall project but I need to make sure all hot works are completed in sufficient time for a fire watch to be set so I will issue a permit which has a cut off time on it. Again, I don't want to give anyone carte blanche to carry out hot works as and when they feel like it within a longer period.
