A group of line managers in our business have recently implemented a new programme of risk assessment, training and auditing in a particular hazard area. (Logistics yard with road and rail interactions). Ringing in their ears is my advice that lovely documents on the shelf are great, but that they mean nothing unless the programme actually works on the ground - emphasising in particular that the requirements of the law put the onus on the company managers to effectively demonstrate to a regulator / inspector that they have taken all reasonably practicable steps in the management of the risks for the operation. (Providing the proof of the pudding so to speak.) The managers are therefore now intent on not just engaging with the shop floor staff but also being able to provide demonstrable evidence when required of how they know that the programme actually works for the colleagues exposed to the hazards on the shop floor. (So far so good.) However - In order to test their new programme, the managers have asked for H&S support in doing a mock accident investigation - to see in practice if their plans are effective and to see if they could put their hands on a good range and quality of evidence to be able to support a claim that the managers concerned had indeed taken all reasonably practicable steps. I'm therefore looking for resources to help in designing an appropriate workshop / training session for these managers. Can anyone point me in the direction of useful resources for this?

ROSPA sell a video on this that you might find useful. As far as I remember it shows an accident (someone falling off a ladder?) and the questions that have to be asked about it.
