Hi, does anyone have any good examples of a 'prompt' style template which managers can use to, not fully investigate, but to prompt questions such as those laid out in HSG245 (which look at people, competence, management etc.) to further understand accidents / near misses / incidents etc. that they would be happy to share?

In my experience something relatively simple works best - ideally a one or two-pager for front line managers, detailing the facts of what, where, when, why, etc. The safety person can then use this as either a starter for 10, or if it is a minor incident to close-out the incident via the form.

Most organisations operate a three tier system of Immediate investigation, local investigation and formal investigation depending on the severity or potential severity of the event.
