If you were building one from scratch what would you want to be able to get out of it? I say accident etc. as I'm including near misses, hazardous conditions / behaviours etc.

buzz, I could be flippant and say data...any system to be really useful must be easy to use, reliable and provide the salient information you need. Like all systems they are only as good as the data you put in. Unless you have hundreds/thousands of events in a year a simple system is ideal. There are lots of off the shelf packages but in my experience they are either too unwieldy or costly to be of any real use.

When I retire I may devise an accident reporting system which is fit for purpose and easy to use.

Hi Ray, thanks for your thoughts. I was thinking more about 'what' with the data, e.g. what would you want to measure / know / compare etc? Would you want to know about incident type, main injury type, emerging trends and themes?

Incident Number, Name of IP, Date & Time of Incident, Date Reported, Day of the Week, Hour of the Day, Area the IP works, Area where accident occurred, Injury Location on the Body, Injury Type (i.e. Sprain, Laceration, Contusion, etc.), Name of First Aid Responder, First Aid Provided, Name of Any Witnesses, Days Lost, RIDDOR Ref, Reason for RIDDOR, Description of the Incident, Investigation Start Date, Investigation Closed Date, Status of Event.

In the past I have built a database (of sorts) and extracted reports on types of injury, types of incident, location and work activity. This was for a large multi site organisations whose managers liked to compare stats.

It had to be clear, though, as we did go through a few versions before foolproofing it, as much as we could. Although managers should all have H & S knowledge, it was surprising how many didn't understand how to record effectively.

This also enabled us to target initiatives based on these factors.

Year on year comparisons are good, as well as rolling information.
