I have two, but have been created on Visio, but then inserted into word; One is the general process and the other specific with numbered matrix

I admit to being intrigued by a perceived need for a "flow chart" for what is (in general terms) a step by step process? The only decision outcome is to ask "am I doing enough, or do I need to do more" and to set interim and routine review periods?

I don't recall using a RA flowchart myself, but I can imagine a flowchart might be useful in some complex tasks or machinery where the assessment throw ups some 'what ifs' that require detailed study.
Could be particularly helpful in identifying where things can 'go wrong', part of your 'do I need to do more' question.
For example for sawing machines, a flowchart could be useful to remind the assessor to consider 'what ifs', like what if the saw jams, what if the saw disc shatters, or what if the LEV clogs up.
If those happen, need to ask what additional hazards are introduced e.g. untrained person trying to fix the problem when it should be a maintenance person, additional PPE required because of dust etc etc.

I use the flowcharts in the risk assessment procedure for clients management system. It saves a lot of words and simple to understand

the flow chart is for a presentation so that it would be easier to understand to the naked eye, with verbal and practical elaboration as to the hierarchy of controls

Flowchart for managers to assess risk: Steps A - D A. Identify dodgy activities / where bad stuff might happen. B. Look at what already doing (to protect). C. Check all’s working: If NO make it (work) go back to B. If YES continue to D. D. Ask if this is enough (new technology, better systems?) If NO – think of new protections and add to B. If YES – end.
There may be some misunderstanding. Hierarchy is not part of RA process (to me) it is part of the risk management.
