Does anyone have a good risk assessment policy with a good explanation of how to evaluate risk on a qualitative basis. I have found some good policies, but nothing that really explains the process of evaluation.

I have a good risk assessment policy - whether it is good for your organisation is another matter. It is better to write your own risk assessment policy and set your organisations own risk criteria.
The evaluation process and how a risk assessment should be made, in term of risk level is still largely a matter of opinion and risk perception by those who undertake the risk assessment(s).

Ann, not really sure if I understand your question but here goes. A qualitative RA is one where words are used instead of numbers (quantitative) which is probably preferred by most h&s practitioners. How you go about a RA, as per previous post, is very much down to your preference. You can use both types if it suits.
For more information I suggest you check out the HSE website, which I'm sure will dumbfound you even more.
