Hi,
Near Miss Reporting is very helpful to identify possible incidents.
Nobody like telling tales on there work mates, but this is a cultural issue and social issue.
So by removing this behaviour from the work place you will have much better results.
I would suggest a quick, (2 hours) Health & Safety Training Refresher (give each a mock Risk Assessment to do) with your most knowledgeable staff member and explain the benefits of Reporting Near Misses.
Get them involved and encourage and empower them to look after their own set area.
Create (if not already) a Safety Team. Involve the most reluctant staff members in this and get them to hold regular 'Tool Box' talks at start of day/shift, this will empower them and make them aware of their responsibilities for work colleagues as well as them self.
Encourage and praise them when they take this task on.
Just my suggestion, but it should help.

If you're working in a place where people can be sent home (repatriated) for not having their pe each morning... Don't expect your teams to report any failings by themselves or the friends. Sometimes the wider cultural and economic issues can have a major impact on workforce engagement with any safety initiative.
