In my experience you would be better off building your own template and working from that. All the off-the-shelf ones are terrible, and whilst some of the smaller businesses will accept them, most companies will send them back for revision.

If you need this to satisfy the (ever increasing) demands of construction clients or tier one contractors, I have looked at HANDSHQ with my very critical eye, and it seemed to offer a cost-effective solution fit for THAT purpose (ie beaurocracy for the sake of it.). The demo was worthwhile and the responses to my questions and challenges were good.

If for in-house, I would do as others suggest and devise your own simpler and more useable solution, more in line with HSE guidance.

There may be very little variation needed to make each 'site specific' - the whole thing doesnt have to be re-written for every project, but it's still a pain for someone to do 3 of these each week.
