We are a UK based company but have had an incident offshore in Irish waters. I understand that I need to report the incident to the Health and Safety Authority (the Irish equivalent to the HSE). The question is do I need to report it to the HSE under RIDDOR as well.

The simple answer is No

As regards HSA, when you say offshore do you mean at an offshore installation? Different reporting rules there than say a fisherman working in Irish waters.
