I have prepared many a report in the past assessing capibility versus requirements for a task and any reasonable adjustments, but for certain conditions i thinks this is a little over the top. i am probably looking for a simple aide memoire style form that can be used where there are relatively few adjustments to be made, such as considerations or questions to ask along the lines of incident or episode procedures for the individual, periodic lone working, work environment (sharp edges etc).
