Hi, I am looking for some help with regard to a Stress Depression Alcohol Addiction risk assessments. It is the first time I have been involved in this type of employee related issues and I am wondering if anyone can send me a risk assessment template for this. Obviously I have sat and had a one to one meeting with the employee and covered any issues with medication etc but from a company point I thought we should probably have some form of document to put on file to physically show that we are helping the employee.

I have a stress management risk assessment using the HSE's Standards Management approach which might work as a starting point in developing your own which is more specific to your workplace.

Not had to deal with this thankfully but I wonder if this is something thay you should be in consultation with HR and your Occ Health provider with?
