An incident occurred recently at my place of work where a Load of approximately 2 plus ton fell from a Crane hook as it was being lifted, thankfully no one was injured. Those involved with the lift (three supervisors), filled out an Incident near miss report. They stated that two of the four strops used had come of the hook, (Quote the hook guard had seemingly given way due to the pressure of the strops resting against it, end Quote) Would this Incident as its been reported as equipment failure require reporting under RIDDOR , would the Hook Guard need to be inspected by a competent person?. They continued to use the Hook after the incident, and its not had a physical inspection by anyone apart from those involved with the lift and the Manager who deemed it safe to use. I work in a Marine environment we are required to recover Navigation Buoyage onto the Deck of a workboat by using a winch The set up is Nav Buoy Shackled to a chain which is then attached by another shackle to the Anchor. All shackles are SWL marked. They have never been inspected by a qualified competent person, all our shore side lifting shackles strops etc are in inspected by a Competent leer qualified engineer at regular intervals. I believe the Shackles used on the Nav Buoys should be inspected by a qualified competent person at regular intervals as a load is being applied to the shackles when recovering the Buoyage, but my manger tells me these lifting shackles don’t require inspection as in His words we are not lifting with them we are dragging the anchors aboard over an open Bow Door. Am I right in saying the lifting Nav Buoy shackles require inspection as they are bearing the load of the anchor and chain when the buoyage is being recovered from the Sea bed? if so how do I convince the Manger of this? could this be reported to the HSE?. Any Advice would be most appreciated its only a matter of time before somebody is hurt or killed there have been numerous accidents such as dropping a Boat on someone luckily it was a small boat and caused only minor injury. The reason for the Boat falling from the workboats HIAB was the rear lifting eye failed and broke off the Boat. the Lifting eyes had never been inspected.

Yes this will need reporting. Howevr you may find that it is reportable under The Merchant Shipping (Accident Reporting and Investigation) Regulations 2012.
This will probably be investigated by the Marine Accident Investigating Branch (MAIB) part of the Department for Transport based in Southampton.
You are correct that all lifting accessories, davits, lifting eyes etc. should be subject to thorough examination based on a written scheme.
